Connected care: Transforming care coordination in the UK

A new report, prepared by the Partnership for Change, highlights why we need to focus on the fundamental need for all patient communities: good care coordination.

The Partnership for Change is a collaboration of ten leading patient groups brought together and funded by Pfizer. The collective power of the partnership provides a unique opportunity to identify common challenges faced by different patient communities and share these issues with healthcare service providers and government so they can be addressed for the benefit of patients across the UK.

While the NHS is rightly considered one of Britain's greatest achievements, recent reports have shown that the public's satisfaction with the service has reached an all-time low.1 Across each of the ten patient groups, there has been feedback from patients whose frustrations have been borne out of poorly coordinated care, with patients frequently needing to retell their stories, chase up referrals, and advocate for themselves in a convoluted care system. That's why this report calls for patient experience and care coordination to be central to any future plans for the NHS and to be incorporated into the new NHS 10 Year Health Plan.

The 'Connected Care' report makes four recommendations that should be considered within the new NHS 10 Year Health Plan.
